    DECLARATION

    1. That I/we wish to subscribe to the aforesaid Services provided by EzCab Sdn Bhd (197870-T) (hereinafter called "EzCab") in accordance with the applicable Package; and hereby agrees to accept joint and several liabilities for all amounts arising from use of EzCab Sdn Bhd Services; and hereby consent to and authorise EzCab to verify with and/or disclose to any party deems fit including but not limited to Agensi Pengangkutan Awam Darat (APAD) and/or any authority or any party whatsoever, any information concerning or relating to me/us and/or the conduct thereof for any purpose which EzCab deems fit without notice of the same to me/us.

    2. That the information given here is true to the best of my knowledge and I have not wilfully suppressed any material fact. I understand that any offer is conditional until the verification of any or all of the information. Any false declaration, misleading or false information or the omission of a material fact and may render my application invalid or termination of existing access.

    3. That I shall be liable if I have knowingly or recklessly made a false statement or omitted any relevant information, resulting in damages directly or indirectly sustained by EzCab.

    4. That any data/information (including personal data) relating to or arising from or in connection with my/our application hereunder and also information pertaining to my/our affairs whether hereunder or otherwise on this application may be held, and discolsed by EzCab for the purpose of processing this application and providing subsequent services for this and other products and services and/or direct marketing to communicate with me/us for such purpose.

    5. That I/We do not have any criminal records in the past, medical condition which may render to unfit to drive safely or such relating case as well as traffic summons for the last 3 months.

    6. That I/we currently drive with a true & legal driving license, Road Tax, PSV & Kad Pemandu & that EzCab reserve the rights & the authority to terminate & ban such account if being discovered to have an expire or fraud license, Road Tax, PSV & Kad Pemandu or any such records.

    7. I undertake to inform EzCab within 14 days if I am charged with, or convicted of, any new criminal offence; receive a police caution; am disqualified from driving; made the subject of a mental health or sexual offences order; or if my medical status changes in any way that would affect my driving.

    8. I agree that I will obey and comply with all the rules and regulations set by JPJ & APAD as well as any changes made by authority of either party in the future unless state otherwise.
